Doing two, because they happened minutes apart -.-

First, Terraria, pirate invasion shortly after my world went hard-mode. Died something like 20 times before I quit.

Switched over to The Witcher - died to one of those plant-zombie things almost immediately.

Was put in a rock, party got me out of said rock right when Titan used Landslide in my direction and then le poof.

Titanfall 2, activated titan nuke on Rise, ejected, bumped my head, nuked myself.

Still early leveled, <10.
Cleared Valtheim towers.
I was standing on the top of the first tower, at the archer's perch.
Forgot I was playing Skyrim and you survive falls about as well as a chickens egg.
Jumped down onto the bridge.

Jedi Knights: Jedi Academy, in my third playthrough after not playing for like 11+ years. Ran up a tall tree and slashed it with my lightsaber to make it fall and create a bridge across a gap. But... I wasn't looking at where I was standing and the tree fell towards me...

On NieR: Automata, I died via my mech explosion due to a slow moving energy blast from a Medium Flyer robot...

Mine was in portal knights. Just roaming through a low level area to complete an event quest with a small group of buddies. All's fine until I fall through a 1x1 hole in the ground, fall long enough to hear one of my buddies ask where I went, before landing badly injured at the bottom of some dungeon in front of a chest. At first I thought I was real lucky, then one of those stupid bat things that explode when they fly into you finished me.

Three firebros teamed up on me and before I could learn their tells they barraged me with fireballs big and small.
