Got lucky with cache draws and was able to make Levianovo!

Formula Level: 3
Formula Odds: High
Alchemist Level: 3
Attempts Required: 1

Also, another formula that I've been leveling with.

Alchemist Level: 3
Attempts Required: 1

Now to wait some more for crazy AI on one item to go away...

Yes, not me personally, but Hastable (Ze3k) has a record of 51 fails for Raphael's Wings.
I'm pretty sure there are others too, but I can't remember atm.

Highest I have on record is 56 attempts to craft a formula at Very Low. Often though, I think people wait until they level up/organise someone at a higher level to do them if they start failing too many times.
