No, not for sure, but from the scenery, it looked like he was in a church. From how he set his speech up, it sounded like he was talking to God. And by what the writers/directors said about his decisions driving the mythology for the rest of the season, I'm banking on it being God. And for Dean? That's ******** huge, because to start with, he never believed in God. Then, he was forced to believe because of the angels, but didn't want to cause it freaked him out, and would rather believe in just Cas than the whole big G thing. And now? I think he's desperate enough to reach out, especially if that was at the hospital Sam was at. I mean, it'd just be huge for his character arch, so that's what I'm going with.
