This is way of making eye-less faces and seeing cool effects of layering makeup items over each other (if they don't cancel each other out). It's also pretty cheap to try out, and you can experiment with this through the Search function on the dress-up page.
To do this all you need to do is equip the Ghost Skin, and a makeup item! YEAH! THAT'S IT! In the image below I equipped ghost skin + Geisha Makeup + Blue Clown Makeup to create a derp face. ;D

Dead Bird Dress makeup really works well for creating a flat/painted eye effect, but there are other makeup items out there that do the same too. You just gotta find them.
Keep in mind that since you're wearing the ghost skin, if you want to cover up the ghost skin and just make that painted face effect pop out, you have to cover the rest of your avatar's body up with clothes items/gloves/whatever. So this effect works best on avatars that don't show any skin aside from the face. The emphasis of this effect is on the face dudes...

Add on random scarf.
Bam. There you go. You have extra tongue sticking out. It might also look like an open mouth. It really depends on what type of scarf you use and how much of the snake tongue it covers up. But generally A LOT of scarves work with this so experimenting should be good.
It's also possible to use the "snake tongue" pose for "fatter"/more luscious lips IF you have a Little Lucie (EI) or SDPlus Noire doll. For example:
