I don`t know, seems like it works on some avatars and not on others. What I did was to delete it and did not pick a new mouth again. Then I just dressed it and saved, still no mouth. I tried to change it and saved and I still was not forced to have a mouth. The character I tried on is from 2003, I am not sure but the glitch could just be on younger avatars...but maybe not. Or maybe the glitch is just random.
