First things first; it's safe to say that this game was beyond overhyped. Even myself was swept up upon the hype train, until it crashed on it's release date. Let me give you my thoughts.
Be warned, however, because MASSIVE SPOILERS ahead!
The whole hospital bit was really well done, probably one of the best moments of the series. With Volgin's corpse chasing after you and XOF Quiet almost killing you, it was quite the roller coaster.
Kaz' rescue mission was pretty great too, with the skulls at the end giving you a sense of "This is what you're dealing with" on the first mission.
Most missions didn't have any real bearing on the story, but they were ok on their own with the many different ways to complete them all, plot critical or not.
"Hellbound" was really fun, having to rescue Huey and having the new Metal Gear, Sahelanthropus chasing you in all it's horror.
About half of the Africa missions made me sad with all the child soldiers and burning bodies about
Mission 20 is my favorite but is pretty messed up
The story and gameplay merging of the Kikongo vocal chord parasite with Mother Base was good
Missions 28-31 are the climax of the conflict and are real good missions
Mission 18 was too intense
Quiet is really cool and her reasons for not talking are justified and everything she does on Mother Base is for the good of everyone
HUEY IS THE WORST because he is a monster and he literally robs Snake and Miller of their revenge
Speaking of Revenge, WHO?!
Mission 43 broke me apart with the Soldiers humming the PW theme and accepting their fate, the cutscene after is the best moment of the game by far, except HUEY ruins everything

Venom = Medic on the chopper during GZ is so clever but the advertisements made it seem like we were the real boss getting his revenge, but it can be disputed with the symbolism that BB and Venom are one in the same

The Idea that the more juicy story bits being on cassette tapes sounds good, but I feel mixed on how it was executed
Code Talker is probably the best character in the whole game
Zero isn't the mean guy he's made out to be, because he sets up the whole Venom Snake project just to protect his friend, Big Boss and help him get his revenge, but the sad thing is Big Boss doesn't know that Zero did anything to help him, and in MGS4 kills him without the knowledge of it, so he's still hellbent on killing him when in actuality Zero wanted his friend back
The Paz sidestory makes me really sad and upset but it's really cool on how Kojima Productions pulled it off so smoothly
BUT after all of that the game overall felt like the ending came at the middle of the game and is disappointing that it leaves too many threads loose
Gameplay is GREAT and can easily distract you from the glaring plot issues
Overall, You should get this game, but don't expect all the answers to be revealed.
I liked it a lot, but not giving it a score because it feels incomplete, but you should still get it.
Also, MGO looks really good so I can't wait for that.
