Either whenever a string breaks or if the strings sound dead.

I typically wind up restringing every month or so, so long as I keep my guitars in their cases and wash my hands before I play them. The strings stay fresh longer that way.

I'd say every 4 weeks. the shortest they lasted for me was 6 days. I wore them down so quick that the A (5) string snapped.

I'm not sure if it was a Fender set-up guide or what, but I read that if you feel underneath the string where it comes in contact with the fret and feel a lot of flat spots and nicks, that's a good indication to change strings. And of course as Hy said, washing your hands before playing and also wiping your guitar down afterwards will help keep them in better shape. One more thing to do is play a little bit before tuning to warm the strings up and allow them to expand.
