Schedualing an Appointment Checkup FormRoyal Name: Parent Name: Stage: Age: Doctor: Prev. Medical Card: Note: If your Royal has had a checkup before, please list the name of their previous doctor, as a Royal will remain with the same doctor throughout their childhood, unless you wish to switch, in which case please state so on the form.Emergency FormName: Age: Doctor: Hospital? [Yes/No] Medical Card: (If applicable) Health Issue: Note: An Emergency Health Issue, such as anything requiring stitches, illnesses, or sudden discovery of violent allergies, will count as PD. Don't forget, Royals aren't the only ones who get sick. While there are no checkups available for parents, if something comes up, you may be admitted into hospital care as well as your Royal.Once you have filled out the form appropriately, keep one copy for your records (post it in your journal) and submit the other to the receptionist (post it here). You will be notified when the doctor is available.
