Anything not seen in a picture..or if something in your picture is not included.
Occupation
If your character has an occupation not related to being a shinobi, list it. Could be a blacksmith, or a baker, etc.
Status
If you are thought to be deceased, put that. If your whereabouts are meant to be unknown put that.

Clan Name
Name of your clan, or just put N/A
Kekkei Genkai, Stage Obtained
Name of your kekkei genkai, followed by a comma, then say which stage your character currently has. If he/she has a kekkei genkai but no stage has been obtained yet, put something like unawakened.
Known Chakra Elements
Only put the chakra elements in which your character has awakened (i.e. can use). You only get three, regardless of rank.
Land, Village
Land your village is in (land of fire, wind, etc.), followed by a comma, then the name of the village your character is in. If your character is a missing-nin, still include this information.
Rank, Title
Rank is a letter (A-E and S) that indicates your approximate level of danger or aptitude. Title is jounin, genin, missing-nin, etc.
Special Skills
These should be skills related to being a shinobi
Known Affiliations
Specific people are organizations or villages your character is affiliated with
Dominant Jutsu Types
If your character knows a lot of techniques in a certain jutsu type (ninjutsu, wind ninjutsu, genjutsu, etc.) put it here.
Intelligence
Pretty self-explanatory. Don't give an IQ score or anything, but simply mention how intelligent your character is.
Fighting Tendencies
If your character tends to fight a certain range, mention that. If they focus on speed, mention that. Etc.
Dreams
What does your character aspire to? What plans do they have for the future?
