Shun would find a small memo in his e-mail to please fill out the following form and ask for June Liu at Humans Resources if you have any questions thank you!! The attached form was a myriad of numbers and symbols that made no goddamn sense because they never do and paperwork is horrible.

Shun checked his email. Stared at the horrible mess of a form. Then went on to ignore it for the time being to work on other things.

He got a second e-mail about three weeks later. It was lengthy and well worded but essentially boiled down to:
Hi everyone,
I saw some of you did not fill out Medical Form W-208B and this is your reminder that all forms are due by June 1, 2016 thank you!!

Shun stared at his screen as he listened to that second email. With a heavy sigh, he went and dug up the original one. And once more it was just a mess of numbers and words and he could already feel a headache encroaching. He tapped his fingers onto his desk, torn on what he should do. His pride told him to not ask for help.
But.
Damn it was going to take forever for him to get through that on his own. Grumbling to himself, he got on up to head into June's office, where he knocked once.

June's office was not so much an office as a converted conference room that had way too many papers, cabinets and small knick knacks littering the single desk in the room. Two computer monitors were sat on the desk and behind them was one June Liu.
Upon hearing the knock she rose to get the door, peaking her head out before saying, "Oh! Mr. Kuroda yes of course please come in." How had she known his name already? She just knew these things.
She welcomed him in and gestured for him to sit on one of the two worn down chairs that sat in front of her desk (the kind with the uncomfortably scratchy seat cushions; the department was not overflowing with cash for nice chairs). "What can I do for you today?" she asked, adjusting her glasses.

God this room was a mess. How could June find anything in here? Shun twitched slightly as she called him out... somehow she knew his name already. Slowly, he entered the room, then made his way to take a seat in front of the desk. He made no complaints about the comfort level of the chairs. He wasn't here for that.
"The damn medical forms."

While he got comfortable she went to sit at her own rolly chair. She folded her hands on the desk, looking at him expectantly.
"Oh! Yeah those can be so annoying," she agreed as though she hadn't been helping people with them all week. "Are you feeling better by the way?"

"Yes, yes I am fine. Or else I would not be here," he grunted. Shun folded his arms over his chest, looking her over.
"So. The forms."

"Oh that's good," she told him. She actually still didn't know the nature of his injury beyond the bandages on one of his arms, but she wasn't a prying sort.
She stood up and shuffled to a cabinet, thumbing through files until she found the correct copy. Returning back to the desk she turned it around so he could fill out what was needed. "Here you go!"

Shun just sort of stared at the paper on the desk, furrowing his brow.
"Ah... right."

She knew that look well.
"Start with box 1 and then work your way through the numbers," she pointed to each of the boxes and the numbers that indicated the order the form was supposed to be filled out in.

The Sergeant furrowed his brow as he stared down at the sheets. God it was just a mess, and his little problem was not making it any easier. He hated to admit what his issue was, but it looked like he was going to have to.
"Er... I... am severely dyslexic. That is why I needed help..." he said. A hint of resignation in his tone.

Her hands went to her mouth, covering up her blush. "Oh! Oh gosh I am so sorry." She fumbled with her jacket a little bit and continued, "I should have known that."
She stood up and rolled her chair around to his side of the desk, placing it next to his with a small bump between the edge of her chair and the leg of his. "Okay. Box 1A is your name." She moved her finger over to the indicated space.

Shun shifted in place again, gripping the pen.
"Is there a way I could fill this out digitally with assistance? My writing is... prone to mistakes."
If not, then. Well. He hoped June would be able to decipher his s**t.

"Oh. Yes I think the pdf I sent you was form fillable ..." She crossed back over to her side of the desk (she tended to scamper like a mouse anyways) and clicked a few things, then typed something at a speed no human should have been able to.
"Um, yeah! Looks like it. If you want I can read it to you and you can tell me what to write?" She ignored mentioning that she could have written the form for him too. She tried to keep the handwriting thing under wraps so as to not freak people out. Most people didn't want to know their paperwork lady could copy their handwriting.
