My mom keeps saying it's OK to have girlfriend and be a girl, but then says really rude homophobic things behind my back.

She even sometimes talks bad about my girlfriend's appearance.

That's really upsetting to hear. You should definitely sit her down and let her know how you feel about the matter and make it clear that it needs to stop. Make sure she's fully aware of how it hurts you. I don't know how close you and her are, so forgive me if that's stupid advice. That's just what I would do.

Either she's ignorant, or she's purposefully doing this. Knowing parents, she's most likely ignorant.

If people don't realize something is homophobic, they don't see the problem with what they're saying.

I agree with A Spoon Full of Cinnamon; sit her down and talk to her. I know by experience that mentioning something as it's said is seen as confrontation.
