PLEASE NOTE : AS OF NOW, EXTREME STATUS IS TERMED AS "APEX" FROM CAPCOM. So monsters go from Frenzy to Apex mode.
Monsters that are infected have a condition where they overtake the infection, much like how players who get infected get a boost after dealing enough damage to the monster.
"When a monster overcomes the Frenzy they enter what is called Extreme condition. They get insanely powerful, most attacks will bounce off them, they get fast and erratic, elemental damage/status damage won’t penetrate them, they are immune to traps, etc."
When monster's become "extreme," they essentially become invincible, even weapons with ESP(Mind's Eye) cannot penetrate the monster. That's where Wyvernstones come into play, they're basically buffs that last a set period of time, and can be reused constantly, though a cooldown marks when you can reuse it.
"A Wyvern Stone can by used over and over again, but there is a cool down period. It’s like sharpening your stone, you do a single swipe to coat your weapon with its power and you’re good to go. A gauge is shown to let you know when the effect will wear off. Then you have to wait a few min. after that to use it again."
The last bit of information is further equipment enhancement. After defeating an extreme monster, you have a chance at gaining extreme orbs. The information is still kind of convoluted and I have yet to fully grasp the limitations of them, but what I can say is that they basically push equipment that has been maxed out, to further heights.
"The second system takes advantage of a special Extreme Orb you get when you defeat Extreme monsters. You can take 3 of them along with 3 rare orbs and an item to give your rare 8+ final form weapons one final boost (Large Extreme Orbs which are very rare are needed for weapons with a rarity of 10, while normal Extreme Orbs are used for rarity 8-9). You get 3 choices (you can change it later but it’ll cost you more rare items):
Attack boost (raw power + 20)
Defense boost (defense bonus of +60)
Health boost (after every few attacks you’ll restore a little health)"
