Lead Instructor/Instructor Evaluation Form
This form is to be used by the Head Master or Master of a given element to evaluate the performances of the current Lead Instructor/Instructors of that element. This form shall be completed in full and submitted in the appropriate thread along with a copy being sent via PM to the Lead Instructor/Instructor being evaluated. An evaluation will be completed once every month for each Lead Instructor/Instructor. Please be honest in each of your evaluations but also try to focus on constructive criticism so that each Lead Instructor/Instructor can improve as needed.

Please use a rating system of U for Unsatisfactory Performance, S for Satisfactory Performance, and E for Exemplary Performance. Please also include any constructive criticisms with each field on where you believe the Lead Instructor/Instructor needs improvement.
