Student Evaluation Form
This form is to be used by the Lead Instructor of a given element to evaluate the performances of the current students of that element. This form shall be completed in full and submitted in a PM to the Master(s) of that element along with a copy being sent via PM to the student being evaluated. An evaluation will be completed once every month for each student. Please be honest in each of your evaluations but also try to focus on constructive criticism so that each student can improve as needed.

Please use a rating system of U for Unsatisfactory Performance, S for Satisfactory Performance, and E for Exemplary Performance. Please also include any constructive criticisms with each field on where you believe the Lead Instructor/Instructor needs improvement.
