Well I always like 2-weapon sneak attacking.

Leap Attack+Shock Trooper+Power attack.

at 10th level, you can cause an extra 40 damage without taking any negative to hit with a two handed weapon.

Okay, kinda costly, but...

Get yourself a ring of X-ray vision, a brilliant-energy equiped crossbow (or bow, or whatever ranged weapon you prefer.)

Now, in the hands of a Rogue this means that you could sneak attack foes though walls.

I find that a half dragon with gauntlets of ogre power is great, you can have 26 str at first level and when you can buy the gauntlets you can get 28 str.

For an epic druid: get colossal animal form and magical beast animal form, this allows you to get terrasque animal form.

Necromancer(opional)+Animate Undead/Create Undead(Important)+suicide spell etch as a rune(also opional but maximizes effecentcy)+Destructive Retribution(Important)+Magic mouth(insult purposes)

In essence, with a minor army or at least a shock force or skeletons. Send them in, if they have a rune that causes them to explode, great, if not still good. Seeing that they are skeletons and don't care whether or not they survive the battle. Have the skeletons self-destruct, when they die (if there are any surviving enemies) the effect of Destructive Retribution will kick in. When the skeletons die it releases a 10' burst of Negative Energy upon its destruction, dealing 1d6 points of damage plus an additional 1d6 per 2 HD (Ref 1/2, DC 15) twisted . So the bigger the skeleton the better the bomb becomes. If the magic mouth was cast on it, have it say to the effect of: Live for the swarm, Worth it!, I regret nothing!.

Monks with Vow of Poverty are always good. Cliche, but good.

Just liked the combination of the Scout class and the Prestige class Order of the Bow Initiate. Skirmish giving additional d6s worth of damage, plus the single shot Ranged precision damage adding in d8s can possibly have you rolling off 8+ damage dice.

20th level rogue + Greater Invisibility + Polymorph (Form of a 12 headed Hydra)

You get 12 bite attacks with a 10ft reach. Since you're invisible, all your attacks will be sneak attacks. If all your attacks hit (you'll have a +21 to hit with each bite, and each bite is 2d8+6 damage), you'll do 24d8 + 120d6 + 72 damage. So that's a minimum of 216 damage, and a maximum of 984 damage.

Crazy, huh?

What book is that in, because I'm a scout in one campaign(in real life) and I've been looking for a good prestige class.

Side Note: He's an elf named Enialis "Erny" Keebler

Another good combo for druids:
Weild oversized weapon(arms and equipment guide)
lets you use a weapon that's up to one size bigger than you

shileilegh(sp)
a spell that makes your quarterstaff or cudgel +1 and strike like it's one size bigger

best when used with super-huge creatures

My personal favorite is a set of two psionic classes and a Psionic prestige class
Soul Knife and Lurker, I think the soul bow or some such thing is the name of the prestige class. Basically all my weapons have sneak attacks and I have ranged attacks with them all, they are psyonic sneak attacks that work against undead constructs and I can add magical properties to them such as bane and that is just on the melee version of them, ontop of that you can then turn those weapons, which by the way you "Forge of sheer will" using PowerPoints but you can shoot/throw making it to where you need no magic weapons no magic bows or arrows, only magical armor, unless you become a monk as well. Bear in mind all of them are based off of intelligenece so the stronger the int the stronger ALL of their Psionic powers are, and the get manifester levels to boot, god I want to actually play that hehehe
