I always assumed that you would just wear a mask and the skull was still there, smoldering beneath the mask, but it was an airtight mask that wouldn't let the magical fire free.
Oh well. I'm sure by now it's destroyed all the nerves that would be in there, so you're likely not feeling any pain any longer. I suppose that's what happens when you're hot-headed. lol
