Why yes, Shepard does indeed dress like a lady, and he takes no great pains to hide the fact that he does enjoy it. Already gifted with a lady-face and all around feminine body, the only thing keeping Shepard from being an actual woman is the lack of lady parts and pretty deep voice. Every now and then, when feeling like taunting people, he'll refer to himself as a woman and use female pronouns, but otherwise the mannerisms he displays are completely in the masculine realm.
He has as many different pretty female clothes as he's allotted, and even wears make-up whenever possible. He'll flirt with both genders shamelessly, even going to so far to call whomever he wants to be particularly cruel to his "boyfriend" or "girlfriend", but Shepard isn't really the commitment type...yet. (He's actually looking for that special someone, but he'd rather stab himself in the throat before saying so). Any attempts to mock his choice in dress or lifestyle are either completely ignored or returned with unnecessary hostility, often leading to brutality.

Calling him catty wouldn't be a stretch of the imagination as he likes to b***h and squabble with the best of them. It's not so much being blunt as it is actually being mean, as even when someone does something right or correct there's always a way for Shepard to let them know they've done it wrong and he clearly would have done it better.
The only people safe from Shapard's comments are those who have power over him and even then he's not afraid of snarking...much. He keeps his tongue in check when the situation calls for it though the minute he's out of earshot the ranting, raving, and general backstabbing comes full circle. Rumors are like the spice of life and he considers himself a chef, ready to pepper situations with more trouble than there needs to be.
Whelp ( or ) Full speed ahead! ( and ) It's not my problem
Looking before he leaps is not something Shepard does, because it's a complete waste of time and he's got better things to do than 'plan' or 'pay attention'. The concept of fear isn't foreign at all to him, he knows what it means to be scared or intimidated, but Shepard generally looks past that and goes on out to do what he wants to do.
He's not afraid of getting hurt since he's not a girl (even though he enjoys being girly), but he doesn't exactly like pain either. He spends most of his time looking back wishing he had the power of hindsight to correct lots of his adventures, but they're all worth it to him. If people get hurt it's because they messed up, not him, so his concern for others as a result of his neglect is limited.

Shepard is at his best (and nicest) if he's allowed to be doing something to involves feeding others. Having trained to be a cook, or having wanted to be a cook before crap hit the fan, he likes to see others eating his food and enjoying themselves.

Unwarranted advice
Otsu knows best. There's really no point in arguing that matter because he's right, you're wrong, end of story. He's super old, has been there and seen that, and you're mistaken if you can tell him otherwise. He knows what you've been through (even if he really doesn't know) and therefore his choice is the best one to be made.
His advice is always handed out at the worst moment in time and is usually late too, if it's actually worth anything at all.
I don't approve of you
Otsu hates pretty much everyone and everything that has to do with making themselves look like something they're not. He's a simple type, an elderly type, and prefers things to not be flashy or shiny or whatever else they want it to be.
The fact that only Shepard can hear his voice downright makes him despair, and he spends as much time as possible reminding his partner that he's a deviant and needs to clean up his life style if he really wishes to become a decent hunter or productive member of society in general.
Back in my day
Waxing poetic, anything and everything has a story because Otsu is going to tell it and everyone is going to listen. He remembers a time that has absolutely nothing to do with the case at hand but it's important so you are going to listen and like it. This goes hand in hand with his unwarranted advice since the advice usually follows on the end of a story.
Why did your human character choose to become a Hunter?
Before Dues EX, Shephard wanted to be a cook, because the outfits the girl chefs wore were divine.
Also.
Shepard wanted answers for questions that no one was willing to answer. He was getting tired of being told he was seeing things and that his paranoia was at fault for his "lifestyle choices", even though his liking of dressing like a girl had nothing to do with paranoia (and more to do with liking dresses).
Mostly though Shepard wanted a place where he could dress like a girl and kick some serious a**. Also, maybe they needed a cook and he really, really liked to cook.
Weapon Ability
Kama-Stuck: Upon being hit by any part of the weapon, the victim temporarily loses their senses due to their face being 'wiped'. While they have not literally lost their face they are given a very brief sensation of having done so and it leaves them vulnerable to attack.
