Of course the Accept or Decline option is most often used in the Arena you can also simply challenge a lower level ranked adversary by starting the thread.
The test is decided on whether the challenger uses adequate T1 based roleplay, the more one posts with advanced details, the more they are likely to Rank up. Depending on what I see, they might only have to take one Ranking Test.
You can edit what rules and setting and conditions you would like in it, but no godmodding is ever allowed. Remember the T1 rules when posting, you can also decide who goes first. The amount of posts in the battle are up to how many that you want to post before finishing the battle, if there is no clear winner, the match is a draw, nobody has to kill eachother, if one does die, they are not eliminated from the real story.
