No. Creators of summons dont need to apply for them. Also, I forgot to ask. Is your character a summon specialist? Because you cant have a sage mode unless you are.

I know. But as a general rule, are you allowed to summon two summons at once? Like If I wanted to summon Gamakichi and Gamatatsu. ((Wrecker and Relic))

RP sample: Heki jumped back as the ninja he was fighting, an Inuzuka, lunged at him. It was unbearable, fighting two creatures at once. Everywhere he went, that stupid mutt would follow, causing him to constantly be on the move in the battle. His opponent, a lowly chuunin, wasn't much different. Hyperactive fools. Hekiyume sighed as he jumped yet again, dodging a charge from the ninja's dog. The dog was about the size of Akamaru (shippuden), and was quicker than most. If something wasn't done quick, Hekiume would end up becomnig dog food!
His sharingan eyes activated, Hekiyume looked at both of his arms, which contained numerous tattoos. He had wondered many times during the battle whether he should summon the poisonous creatures that resided in a land far away, instantly being summoned in a manner of seconds. He didn't want to, this inuzuka didn't deserve to face them, but it seemed more and more likely that he would be forced to. This moment of distraction kept him still for a second, a second he would soon regret. The dog, after transforming into the master using the "beast human clone" technique, was now chasing Heki and before he knew it, the dog was seconds away from uchiha. "s**t. Guess i'll have to use it......"
Pressing his index and middle fingers to the tattoos on either side of his wrists, Heki outstretched his hand, his eyes still intent on the dog. The Inuzuka, who was smirking before, now stared wide eyes, for his pet had been stopped in his path. But, it wasn't Hekiyume that stopped him. The dog, in the shape of his owner, was hung up in the air, struggling for breath, as two snakes wrapped themselves around its neck. The snakes hissed loudly in the forest, moving so slowly yet eloquently. Heki's arms were still outstretched, but he stayed completely still. His opponent was stunned for a few seconds before resolving to free his better half. He ran at Hekiyume and his claws started to grow, as well as his teeth. The speed at which he moved surprised even Heki, for his sharingan eyes had trouble keeping up. The ninja clawed at the snakes, cutting them in half, sending the dog free. The canine, now back in his regular dog form, was on all fours, growling at the uchiha who had nearly killed him. Hekiyume, however, merely glared back, pulling out a scroll as his face stayed expressionless.
With a bite of his thumb, he set the scroll on the ground, performed hand seals, and slammed his hand on the parchment. Numerous snakes, from 5 ft long to 50, all came rushing towards the two Inuzuka team, their tails all seeming to come from inside the scroll. It was truly a nightmare, a nightmare that Heki watched. After the snakes were finished, Heki quickly unsummoned them, placing the scroll neatly on his back. He walked over to the bodies of the once proud owner and pet and carried them both, deciding that they needed a proper burial.
