Here at E|TfC, we have a standardized template. Its markup code form looks like this:
Name:
Nicknames:
Approximate Age:
Date of Birth:
Prophet of: (Skip this if you're a blacksuit or a civilian!)
Rank: (Ditto.)
Occupation: (Blacksuits or Civilians must fill out this section. Optional for players.)
Personality:
Likes:
Dislikes:
Powers: (Typically we'd like to see two-three minor powers, or one major and one minor as a Newbie, and then you advance from there. If you're a blacksuit or a civilian, skip this section.)

Power One-

Power Two-
Appearance:
Hair: (Color and style)
Eyes: (Color and shape)
Face: (Shape, any significant features.)
Body type: (General build information goes here. Also skin tone, and any divine attributes not associated with the face.)
Fashion sense: (a general idea of what your character might wear.)
Accessories: (Any significant props we should know about?)
Once you fill out your template, you should place it in an easy-to-find place in your diary or quest thread. This is also the form we'll be using for most RP contests, so using it for your quest will make it easy to enter! Although we've never had an official form at EndGame before, we're sure that in time this system will feel perfectly natural!
When filling out the form, pay attention to what sections you do and don't need to fill out. When filling in a section, be sure to delete all parenthetical instructions. If the section is to be omitted entirely (god, rank, and powers for blacksuits and civilians), you can just go ahead and delete it from the template.
If you have any questions about the template, feel free to ask here or contact the shop mule! Otherwise, live it, love it, and keep it groovy.
