Name: Your User Name
Card Type: Your favorite type
Attribute: What best described you
Level: Your School grade level subtract 5 (assuming you go to 4 years in college). Example - Out of college = 12stars. Example 2 - 4 years in college would be grade 16... 16 Subtract 5 is 11stars... Example 3 - Senior(12) - 5 = 7stars
Trap/Magic Type: Whatever you want
Card Rarity: Check the Gaia Member search for your user name - And based the Rarity on it .. Example - Naruto09129843, Naruto_U, Naruto_rules - is common. Cotton candy condom - Ultra Rare
Picture URL: Your real picture - or - Your Avatar Picture
Circulation: Put: Duelist Haven
Set ID: Your birth month abbreviation (3 letters) / Your birth date with the number zero in front
Type: Your Choice
Description: A description of yourself or whatever you want it to say
ATK / DEF: The attack and defense of your favorite monster card that you used for the Serial Number.
Creator: Your real name or Your nick name
Year: Your birth year
Serial Number: Serial number of your favorite monster card. (you can find the serial number at the bottom left corner of the card, or search for it on http://yugioh.wikia.com/wiki/Main_Page)
