I don't know, but I'd probably say not, because it would still have been their second chance, even if they sort of wasted it.

Well, I'm not sure natural causes count towards a ticket for the game, and most of the contestants seem to be teens and young adults. Unless, you're thinking they come back in the middle of a street and get killed by a car. I'm not sure, then.

No... the thing that defines all the characters that play the game is that they all are somewhat regretful of their life. When you play the game an win, the "points" that you gain while playing are about your growth as a person (That's why Shiki won). So I think you'd be eligible to play the game if you need to feel a sort of emptiness in your life.
Neku felt he didn't need others, Shiki was envious of a girl who seemed to excell in what she wanted to do with her life, Beat felt like a worthless and stupid (there might be something else but I don't remember) a person with no real future compare to his sister... And Rhyme felt like she had no real dreams or aspirations.
That's why they need that second chance. To fill what's missing in their lives.
