Did they ever have relations? They were both Time Lords.
As an old-school fan (and full-time 'shipper), I must give complicated answer:
According to the official party line, no. Due to the fear of an uproar by conservatives if the Doctor were seen to be "living in sin", the BBC had the now infamous "no hanky panky in the TARDIS" rule in full effect.
HOWEVER,
Tom Baker and Lala Ward (Romana II) were dating while working together, and actually were married for a while (apparently, Tom Baker is as hard to live with as the actual Doctor, and they divorced). Some of their "chemistry" is said by fans to have spilled over into the show. The notable moments:
"City of Death": They were seen "frolicking" (holidng hands while running across the
Champ du Mars) in Paris.
"Shada" (ep. that never aired, clips are wandering around the 'net): They seem to be on a "date", punting down a river.
"State of Decay": While imprisoned by vampires, they exchange possibly tender glances and call each other "wonderful" in soft voices.
Now these are anything but hot-n-heavy love scenes, but for us old-school shippers who liked to fantasize about the Doctor's (tormented, always ill-fated) love life, they were as good as it got. You modern fangirls, who get to see him snogging on everything that moves, have it easy.
wink We had to extrapolate our Whovian romance from hand-holding and longing looks.
So, to answer, were the Doctor and Romana in love? I have to say, "If you want them to have been."
