Ok, the story starts off with my girlfriend and I having sex. As per usual, we both climaxed, and we both layed there for a few seconds, enjoying the moment. I pulled out, and the condom was not on me. I looked to see if it was on the floor or on the bed, and it wasn't there. It ended up being still inside here. Now, she said it was a bit scrunched up, but I looked at it and it contained about as much as I usually ejaculate. Depending on the circumstances, what are the chances (exact percentages if you could) of her getting pregnant? This is really important to me because I don't want to cause hardship in her life right now........

It is impossible for us to give exact percentages when it comes to pregnancy because pregnancy depends on many factors. All we can tell you is that pregnancy is possible from the situation that you described. If this happened very recently (72 hours ago at most), then she can take Plan B.
Plan B is a form of emergency contraception. That means it's not birth control. But it can be taken after situations like this one when your birth control fails.
If this happened more than 72 hours ago, then her only real option is to wait for her period. If she is late, she can take a home pregnancy test.
She should also visit an ob-gyn if she hasn't already. All sexually active girls need yearly pap smears, and the ob-gyn would be able to give her another form of birth control (such as the pill, patch, ring, etc.) to use in addition to the condoms so that she has a backup method in case something like this should happen again.

Just out of curiosity, was she on top and sat like straight up on you right before? reason i ask is because i have lost a condom twice inside my girlfriend and both times it was in that position

no, it was more of "her on her back on the bed, while me in between he legs standing up"

They are pretty high if any of your semen leaked out.
I would suggest plan B.
