Nintendogs
and probably friday the 13th on the c64..
just.. the music.. oh god.. it sucks so bad.. it ACTUALLY gave me nightmares.. like.. genuinely.. i would wake up with nightmares of that tune
plus, randomly through the game, when people get killed by jason, u normally hear this shriek, but when i was younger, when i was playing it, for like the first time in like 10 plays this random picture of a cleaver through someones face flashed up on the screen with flashing lights and i almost pissed myself

I haven't played enough games to say what the scariest is, but the game where I had the most shocks was Fatal Frame 2. I often nearly had a heart attack <.<
The games where I was scared most were Fatal Frame 1 and Silent Hill 2. I Fatal Frame I always were afraid that a ghost could attack me every step I went... and Silent Hill 2's Toluca Prison creeped me so out that I REALLY was scared. I mean it.

Silent Hill 4. I got/played it recently, and besides all the creepy horror stuff, having enemies that can't actually die (I.E. victim ghosts) does NOT sit well with me.

Resident Evil 4 scared the s**t out of me the first time I played it.
But right now, I think it's Eternal Darkness: Sanity's Requiem (yes, I know it's a GC game), simply because it's so damn psychotic and creepy.

High School musical the game *eye twitches* also since I only have one scary game, i'll say Resident evil 0. i still have to get the other ones but i always end up playing resident evil 0 during the night which is the worst time to play a game like that if youre a big scardy cat like me.

Same for me The part that annoys me is when you have to have that girl follow you around beating herself draining your heath
