That it doesn't require us to wait a month after its release to play while they "fix the bugs" that they should have dealt with in beta.
PvP
an actual learning curve
the ability to defeat lvl 1 monsters with lvl 1 equipment instead of having to mooch off of a higher character until you have lvl 5 equipment so you can deal with those pesky lvl 1s that they decided had to be "challenging" for new players =_=

gore is the least likely thing that will be added due to Gaias "PG-13" nature even though if you read through the comics they like to push the boundary.
