XD Quick translation -
Admit to oncology diagnosis osteocarcinoma (cancer) Complete bedrest - semi fowoler's position (it's a position, how elevated the bed needs to be) Vital signs every four hours until stable, then every shift change. Diet as tolerated - allergy to all dairy products. Eggrate mattress to bed. IV of 1000 ml of 5% dextrose in lactated ringer with 40 mili equivalent potassium chloride to run at 125 ml per hour - call if the IV infiltrates. Prednisone 0.5 by mouth ever day Ampicillin 250 mg IV piggyback every 8 hours Digoxin 0.125 mg by mouth daily (same thing but with synthroid) Tylenol i or ii (these marks aren't commonly used by doctors as they're apothocary notes; it's a different way of measuring than the pharmacy does, but it stands for grain, I do believe XD;; ) for headache Lomotil i or ii tabs four times a day for diarrhea Demerol 100 mg intramuscular every 4 hours for severe pain May use soft restraints if necessary.
Now from that I have to Kardex all of that information in the proper areas on the Kardex, order an IV pump and tubing as well as the soft restraints and the eggcrate that will go under the sheets, then I have to ask the nurse what the patient can tolerate eating and order that, then I have to add ALL of those medications to the MAR. Which means finding which side and area it goes under.... @.@ And sometimes, write it twice if it has, for instance, a or attached to it. (One or two tabs. I have to write out the order for one, then for two tabs separately.)
Not to mention the marks I have to properly make to cover my a** in case of some sort of legal problem or to just remember that I last left off at since I'm also answering questions and answering phones, and doing stats or calling a crash cart if a patient codes. (XD; Heart stops.)
