It's hard to explain if you haven't read the whole thing... But the idea is that Largo seems to suffer from dillusions that the game/fantasy world is real. at first everyone (including the reader) dismisses these dillusions, but as the story goes on, we come come to realize that Largo's perception of the world is suprizingly accurate, if not a little skewed. If you want me to get all philosophical on you, I would say that the zombies and other things that go on in MT that people just seem to ignore are physical manafestations of bad/sad/angry ideas/events that people in our own world would reather ignore than accept and try to fix. As Largo says we "plan for defeat" so we don't have to think about how it makes us feel.
