The name of this game is operating in the shadows as best you can, because you're all effectively mafia here.
There are three factions. Each faction is made up of two teams of players. (This makes the minimum number of players 12, with the next option at 18.) Each team knows of the other players in their team, but nobody else. This means that half of each faction is completely clueless to the other half.
Each day the order comes down to lynch a member of a specific faction. Failure to lynch a member of the correct faction or failure to lynch at all results in a random kill decided via roulette. (Ready to match some colors, kids?)
Also at the beginning of each day, there will be a diceroll (with a "no faction repeats" qualifier) to determine the faction, and then another to determine the team, that will send in the typical mafia kill for the night. The members of the selected team will be privately notified.
In the day's writeup, the dead will be revealed for who they were as per usual, but the mafia kill will also specify which faction did the killing.
The last faction remaining wins the game.

We're a little unsure about special roles. The game is complicated as it is, but special roles provide more than the magical powers people usually see them as. With that in mind, we believe each faction should have one Cop that can only target members of a specific faction. (Example: Green Cop can only target Red Faction, Red sees Blue, Blue sees Green).
We had also been tossing around the idea of a role that exists specifically to be kept around for a single day after they die, but I'm beginning to question the usefulness and point of such a role.

Things to keep in mind:
-Since teams are unaware of each other, you will sometimes get night kills that look like "Green Faction kills Green Member." XD
-If someone dies and you go "damn," that automatically paints you as a member of the same faction. On the other hand, shouting "******** yeah" tells that faction you're not one of them. This is a very, very cautious game.
-Out-of-thread communication is certainly allowed, but only with your own team members. Think of it as if you're mafia in a normal game.

nobutsrsly this is an interesting spin on it. I would be so down for this.

As far as roles go, how about a one-time role that changes the outcome of the dice roll? Would that be too OP if every faction had one?

I like the way you think.
They'd have to send in the use before the day ends, and it would ensure that their faction was picked next (teams are another thing entirely ;D).
I like this, it makes it possible for each team within a faction have one special role.
I am calling them the Gambler, if that's alright by you.

If you were wondering about the other thing I mentioned before, it was a one-use role that Rhed and I had started calling the Ghost. The idea was that one person per faction each night would be randomly assigned as the Ghost, and if they died that night as the Ghost, then they could stick around for the next day and then be done. Their death would use up the Ghost role for their faction, and that would be that. It's an interesting idea that Rhed had, really, because that person's no longer a threat, and all of their information is out in the open. At the same time, there's no threat to them, so their whole perspective changes and certain things may be easier said. Unfortunately, I'm not sure this is a very good idea, as it's pretty easy to just spew everything you've learned from the Cop on your team and get a bunch of people lynched.

Oh, if it's alright by you guys, Rhed and I would like to run this game together in a few weeks if we manage to work the last couple of kinks out and get approval from a good amount of people.

I am confused by these two points. The first one makes it seem like the teams do know who makes up their teams but not who makes up the other teams. And the second point makes it seem like the teams don't know who their teammates are. And if they don't know who their teammates are then how are they supposed to decide on a kill? And if they aren't suppose to decide on a kill, then who sends in the kill? If one person is assigned the kill roll then this would seem more like a game with three towns who each have a vigilante, someone who is aligned with a team and is trying to kill other teams but shooting blind. Calling them a mafia would be more like team work.

You're getting a little confused over my wording, I think. Care for a scenario?
Let's say that you and I are members of the Green faction. There are two teams within the faction, and you and I are on different teams. We are part of the same faction, and thus on the same side. However, since we are on different teams, we know nothing about each other. Now, you know that LPS and Kath happen to be on your team with you, but you couldn't have any idea that Wylr and Med are on my team. You are somewhat suspicious of me, and think I belong to the Red faction. When the time comes, your team is given the chance to make a night kill, and you all agree that I just don't seem like a part of the Green faction. When the kill is written up, it says that "Cookie, a member of the Green faction, was brutally murdered by the rest of his own faction last night."

-Earthbound: Ness & co and Mr. Saturns vs concerned citizens of Moonside/Happy Happy Village vs Pokey, Giygas, and everyone they ******** up along the way.

As for why they attack their own teammates and making that realistic, maybe you can just make it look really derpy and accidental. Like Ness swinging his bat around yelling DEMACIAAAAA and accidentally knocking a Mr. Saturn into orbit.

Right, but then who's doing the killing when Rhed and I hand down the punishment for lynching the incorrect color? Or what about when we say "everybody go kill a Yevonite?" And what is your motivation for remaining behind the scenes then, if one of the groups is as out in the open as Yevon?
(and my favorite is still France vs England vs Spain, because history kind of went that way anyways XD)

Generally speaking, we've found that having too many specific roles on top of our triple mafia system is going to send everything to hell. Thus, having a role based on death, such as the ghost/gambler.
The color specific cop. AKA the informant, is currently the only actively different role one can have.

Generally speaking (for a second time) we have to place the informant with a vanilla member, and the gambler with a vanilla member. We dont want one team (among each faction) to have everything
