agility- u fall off your stripper pole
thieving -the bag is too heavy for you and you get caught by a guard
runecrafting-the runes combine and shoot fireballs at you

Slayer- Getting attacked, trying to flip the monster over but it's too heavy and you fall down and it mauls you.
Hitpoints- You get hit by the first arrow and you collapse, dying.
Construction- You build a shabby house and it collapses on top of you.
Crafting- You build a statue, then accidentally slip and crush the thing with your tool.
Fletching- You build a bow, show it off, then it snaps in half.
Herblore- You construct a potion, only to have it blow up in your face.
Prayer- You float two inches off the ground, then you are struck by a lightning bolt and you fall, dazed.
I think you should post this on the forums. I'd do it if I didn't have a Hunter cape, because I love it. I'd have to unequip it due to embarrassment. I love my cape, but this should definitely be posted.

prayer SC - You fly up then the wings disappear and then you fall down with a broken ankle.
construction SC - you build walls around you and then the walls wont disappear so you're stuck forever (until you tele out)
