I think what they are trying to say, Nyachi-san, is that "brother" is an important relationship to them. Having someone else call them "nii-san" cheapens the statement, and takes away what is special about their relationship.
Usually, if someone says you are not their brother, it is actually a hint to stop calling him your brother.

 No. Unless Hikaru sais differently, don't call us anything refering to brother or elder brother. We have names, use them.

Haruhi is not Hikaru. Please listen to what Kaoru says, Nyachi-san. It is rude to disregard other's wishes. Clearly, Kaoru wishes for you not to refer to him or Hikaru as "brother". So it is polite to quit calling him that and to refrain from trying to get around his requests.

 Your right, you know what... I completely forgot about them not having any manners. Though I felt I had to say what was on my mind, and I thought they way he phrased it sounded rude. That is not any way to treat a guest.
