I think the parka's a bit much, and not enough variable shading on the green...too much of one shade makes it look like a big green blob.

Blue and white is an excellent color combination. That was my first thought when I saw your avatar. Then I noticed the black. Actually, I noticed it a little at a time... it got me =)

I don't understand the bird....but the rest of it looks fine...I'd say add a white coat to truly capture the doctor persona

It's got an odd style =) but that's not a bad thing in the least. Much simpler than usual, and it's all an obvious color scheme.
The scythe throws the avatar off-balance but, yet again, that works. My compliments to Slim for making a cool avatar =) though yours are usually awesome anyway.
