Last night was a success, if Petiole had anything to say about it. She was so tired, but it was so liberating to get the first shadowing attempt behind her. She had a successful night shadowing the same doctor, apparently they were one of the main night shift doctors and ended up patrolling all over the hospital when they weren’t babysitting sleepy dryads. She knew it was a joke, but it still made Pet want to run for the blanket fort.

Otherwise, it was a calm night, full of meeting patients, quietly observing and offering suggestions on some herbal supplements. She wasn’t fully caught up with the medicine names, but she did know a thing or two about plants that wouldn’t have poor interactions with them. A little mint or lemongrass here and there wouldn’t hurt, especially if it livened up the stale air of the hospital. The doctor later questioned her about some hypothetical patients and aced each one. She was so thankful for another chance from the doctor, she even had an inkling of what she would be naming her child when they came.

User ImagePetiole looked to her hands as she contemplated the question. “Healing magic could be used in a wide variety of manners, though I think that ‘useful’ is a very relative term. If you’re good at it, a touch of healing magic could be used to take the sting from a child’s knee after they fell or to dull the sting of stitches. It could also be useful in extreme situations, where a life is on the line and they need to be stabilized. It could keep your lungs and heart moving long enough that the danger has lessened.” She wasn’t going to mention restarting a heart, she crossed her arms as she thought about it, it wasn’t a magic that she wanted to think about.

“Honestly, it also depends on the caster. Dryads tend to have rituals associated with their magic, so manual medicine is more up my alley but I know a quick spell in a pinch. There are some wounds however, that need some time. A broken arm should be mostly manual, if only because a mistake could lead to an improper setting of the bones.”

Pawsa sat and thought about the question at hand, going over things they had already learned in class about this sort of situation. Raising her claws, she then stood up, cleared her throat, and began her answer. "On a normal basis, no it is not without following certain guidelines. That goes against the patient's rights to confidentiality. The only ones who should be privy to their health care information would be the patient, obviously, the doctors in charge of their case or helping to consult on their case, and their nurses in charge of the patient's care. Should the patient have someone they wish to have their medical information told to, such as family or friends, they can fill that out in a form and make it all official." The hybrid yawned and stretched her arms behind her head. "Sorry about that... study night last night. Heh. Anyways... outside of those who have a need to know, or the patient gives permission for someone to know, you can't talk about the details of their case to avoid breaking confidentiality and getting your arse handed to yah."

Krol thought over the question as she had since seeing it written on the board like that. Her first instinct was no and flip the table like it was the end of the conversation. Still there was no way the teacher would give them something that simple so here she was mulling it over while everyone else didn’t even hesitate given the question. Nightmares and daydreams all deserved privacy and it was important not to violate it but when was a breach in protocol needed. Her fins wriggled as she came up with her answer and started writing.

Is it ever acceptable to relay information about a patient's treatment, current health, or future health plans? Yes in extreme cases it may be acceptable to relay private patient information to others. For example if a nightmare’s future plan involves harm to self or others in the community it should not be dismissed as joke nor should it be held in confidence as the end result may end up harming the lives of others. Instead the information should be processed through proper channels so the nightmare in question can get the help they need. It should not be shared with random strangers obviously nor should the information be joked about or discussed over the water cooler in the office. This information is sensitive and very personal and private. There is no need to share it outside of proper channels that pertain to the issue at hand. When in doubt one should consult the hospital policy on such matters and if none exists then talk to the supervisor and find out what needs to happen next.

More over if the patient has a highly contagious condition it may be required to seek out others who the patient has been in contact with in order to get them tested. In a case like this information must also be shared to a certain extent.

Again the cases should be evaluated carefully and those that are found to be a danger to others and the community may require some divulging of information but only what is allowed and only through proper procedure and protocol. Anything else is a violation of confidentiality and unacceptable.

Xochi was enjoying the classes on medicine. They were challenging, and for the first time in a long time she felt like her brain was getting a good work out. Her notebooks were well taken care of, and instead of a scattered bored mess, the demon was cool and functioning perfectly.

The assignment was a little baffling. She didn't understand why they were asking, but it quickly became clear that the answer wasn't a simple 'yes' or 'no'.

Is it ever acceptable to relay information about a patient's treatment, current health, or future health plans? This depends drastically from case to case, but there was a few rules of thumb that can be followed to help physicians understand their role in long term health and safety of their patients and others.

Should a physician learn that a patient intends to self harm or harm others, or that the patient has committed a murder or other heinous deed, it is then imperative that the physician turn this matter into the respected authorities. For self harm, a psychiatrist would be the most appropriate, and in the case of danger to others, either or both a psychiatrist or police should be contacted. If a heinous act or murder is suspected/confirmed, the police need to be contacted.

There is also the chance that a patient has given permission and waivers to speak to family members or close friends about current or future treatments. One should always check and double check these forms and ensure that this information is accurate. Verbal agreement over the phone by the patient is not sufficient in this case, a legal document must be signed giving these people permission. The patient likewise has the option of rescinding their permission. This is why checking who is currently allowed to know sensitive information is very important.

In cases of search warrants, those involved must give the information to the authorities. These are rare cases, but can occur, and physicians need to be aware of the laws each race holds up in terms of legal action and patients privacy.

In closing, there was a few cases in which sharing personal information of a patient is nessesary for the health and well being of others, but these incidences are seen as the minority, not the majority.

User ImageIt is rarely, if ever, acceptable to discuss a patient’s treatment and or information. There are exceptions to this rule, as with all rules, but upholding patient confidentiality doesn’t have many. First of all, if the patient would like someone to know, they’ll have to fill out the necessary paperwork. Verbal consent should not be relied upon.

Otherwise, if a patient is unable to sign these documents, say if they’re in a coma, then only next of kin should know their status and treatment. These include their parents and/or guardians, spouse or children. Also, if the patient is younger, information may be shared with their parent or guardian seeing as they oversee the wellbeing of the child. Information could also be given if others, or the patient themselves are in danger. If they’re harming someone or themselves, or carry a pathogen deemed dangerous, others may be notified such as the authorities, other doctors and possibly even those who they have come in contact with.

Overall, the situations need to be weighed by the medical staff to see if the information should be discussed with someone other than the patient. A break in confidentiality is a break in trust.

User Image Good bedside manner. That was something Pawsa never even really thought about, but then again it had been brought to her attention that she needed to work on some things. From her personal experiences so far she thought about what she was told made good bedside manner, and also what she had been lacking in.

Taking a stand, she started off "Well some things I've been told that are really important," leaving out the fact that she had been told to work on a couple herself "are things like paying attention to your body language. Taking a defensive posture, like crossing your arms, makes you closed off to the patient and they won't feel as if they can truly reach you with their concerns. You want to give them your full focus and pay attention to their concerns. They are coming to you because they are in a place where they need help. If they can't trust their doctor to care about what they are going through, they might begin to feel as if their concerns don't matter and may even stop thinking they don't deserve to be taken care of. And even if you have to tell them something they might not like to hear, be a shoulder to lean on and reassuring. It really can make all the difference to know you are on their side in things."

Nodding as she finished, she took her seat again.

User ImageEliot looked at the assignment and then thought a moment since it was a very serious topic and deserved respect. Especially since it was something he could encounter in the line of work he wanted to go into. He wrote "First I would take notes of the neglect for the hospital records and take care of the patient. While taking care of the patient I would talk them, find out how they came to their current state family problems, illness, emotional problems. Then If the patient was clearly being abused and needed helped they didn't understand they needed I would then follow procedure and call the authorities, taking notes to give them all the information they would need to medically prove the neglect." Eliot bit his lip worried that what he wrote wasn't enough, how could it every be enough when thinking about someone who appeared to be neglected, but not everything was always as it seemed so as a caretaker in any field he could let himself overreact.

            Neglect. Xochi had a soft spot for small ones. This was definitely something that she had to face - a reality that could come at any point in her career. Knowing that seeing neglect was going to upset her was no reason to avoid it. Knowing how to handle the situation if and when it arose was the key to successfully navigating the situation.

            "As a physician, I am a Mandatory Reporter. This means, even if I believe the neglect is unintentional, it is my duty to alert the authorities. While it pains me to not do more, doing what I can is the best possible way to keep an underage patient healthy," Taking a breath, she weighed the topic again, "My first move should be to talk and comfort the patient. They might be able to give me helpful information about their living situation. It could be a case that the neglect is... Not intentional, but a product of a busy life." Was that possible? Were... Were there parents so busy with their lives that they could forget the well-being of their children? Xochi resolved to give Lunest and Diamandis big hugs that weekend.

            "Once I have a clearer understanding, alerting the parents comes next. I can tell them what I have seen, though if there are any other signs that the patient is facing abuse, informing the parents that there is something amiss could cause further complications, especially if authorities get involved."

            Whoops, there she went and rambled again. Clearing her throat, Xochi began to speak, "In conclusion, it is always the physician's duty to inform the authorities. Whether to inform the parents should be taken with extreme discretion."

"I think that the obvious thing to do would be to sedate the patient," she said, "but I would be reluctant to do that. I might not know how much sedative to give them, or how it would affect them. Whatever is causing the fever could react very poorly to a sedative. A better option might be to use magic to calm the patient down from a safe distance. Alternatively, you could instead try talking to the patient slowly, calmly, and gently. Don't make sudden movements, don't raise your voice (unless you need to call a nurse or a doctor over), just...talk to them. Let them know that it's going to be alright and that you care how they feel. Ask them what's wrong, and go with it, whatever it is that they're saying. I would suggest setting up a shield charm early, just in case they do get physical and you need to protect yourself or the other patients in the emergency room." Her tail flicked. "Oh yeah! You should get them out of the emergency room soon, so that they don't attack the other patients. Get them attention, take them to a private examination room or ward, and if they attack you, don't fight back. You could seriously hurt them."
