I got this after I got a character to level 12 and then did 2 origin stories.
It's cumulative.

It's supposed to be cumulative, but it's glitched for PS3, something to do with the latest updates. I've played it all the way through at least five times, so either it's glitched, or I'm the worlds suckiest Grey Warden.
I've heard that I can uninstall all my data and DLC to fix the problem, but I'm not that desperate. XD

You have to be the one to kill the darkspawn, not your party. If they kill them, it adds to that files kills, but not your overall kills. Best idea is to try doing a solo-run as an arcane warrior mage. They are one of the best tanks, especially if you power-level magic and constitution.
X3 I eventually just cheated. I had to completely reset my PS3 to factory settings a few months ago, and that sucked, but I had the opportunity to replay DA: O without installing any of the patches.
Basically, there used to be a glitch where all of your party kills would count towards the trophy (that or all kills, not just Darkspawn, counted for your character. I'm not sure.
Either way, I got the trophy, then installed the patches.
