Planned Parenthood would be able to diagnose, treat, and discuss many fertility issues. However, if they do not see a problem and you two have have actively trying to get pregnant ("actively trying" means keeping track of her cycle and having sex before and during ovulation) for more than a year without success, you two might need to see a fertility specialist.
I doubt it'll be free or low cost like the clinic though since having a baby isn't something people actually need in order to live or be healthy. And don't forget if you can't afford health insurance or doctor visits, you might not be able afford a baby either.
Also, if she is only 19 years old, not seeing a doctor, without health insurance, and without money then you two should not be trying to get pregnant now let alone a year ago. Babies born to teens have more health problems, and women need to regularly see an ob-gyn before they even start trying to get pregnant.
