Chunin exams at first, and then through kage's promotiong them atthe appropriate level of skill. Though there are a few exceptions to this rule.

Okay to answer your question you can create just about as many puppets as you like just keep in mind that they must be sensible and cannot have outrageous abilities that basically make them human puppets. However its not so much how many you can create its more about how many you can control. The puppet techniques are pretty clearly labeled with what rank you have to be to learn them. Its the chakra strings technique that labels the number of puppets you can control. So does that clear things up for you?
