Tatsuman Meta_Fish People that use top tier characters only, and don't pick certain stages because they'll have a disadvantage. Most of these people will only pick Final Destination, so it's "fair". Also known as Tourneyfags.
FD is possibly the least fair tourney stage there is. Pick Fox and shoot lasers. >.>
Then run. Then do it again...
These are the only stages that are banned.
* Brinstar Depths
* Flat Zone
* Great Bay
* Icicle Mountain
* Hyrule Temple
* Yoshi's Island N64
* Yoshi's Island (Pipes)
* Venom
* Fourside
* Mushroom Kingdom I
* Big Blue
* Mushroom Kingdom II
And then Search GimpyFish on Youtube for an answer to your "only top tier" comment.

Also if you know the right tactics with certain characters you can get many free k-os. Such as Luigi on great bay temple. He can back throw you under the main platform and well you die. Or even Fox on Final Destination. You can run to one side and laser spam. Then if they approach you simply run to the other and do it more.

Not sure
Does anyone know?
I get midnight most of the time, but that's not clear enough for me.

Flat Zone's kill lines are unbearable. Just too damn close. Marth, Jiggly, and the Mario Bros. can get unavoidable easy kills here.
Icicle Mountain, I'm not really sure. I don't know anyone who actually likes the stage, so that might have something to do with it.
Temple is banned because of Fox and Falco's ability to laser camp there and be absolutely uncatchable without counter-picking Fox or Falco.
N64 Yoshi's Island is banned because of the ability to stall the match indefinitely on the second cloud.
Yoshi's Island is banned because of the Wave Shine. It gets an unavoidable kill on half of the roster.
Venom is banned because of it's effects on certain characters. Captain Falcon and Ganon, for example, can't use their Down B attacks on the ground.
Fourside is Falco's heaven. Characters that can walljump can live much longer than characters that can't. Plus a Falco spike here pretty much equals death.
Mushroom Kingdom is banned because of the Waveshine, same for Mushroom Kingdom II
And characters like Bowser or Ganon have a huge, huge, HUGE disadvantage on Big Blue.
