What does the layman know about Insanity, if at all? Since most regard matters of the Core with superstition, would they think of it as like their version of the Black Plague/leprosy and avoid someone who has the physical symptoms?
It is still relatively new -- last year, beings from Christmas were infected with it. They acted strangely, and they seemed to be composed of grays and fog. The general belief is that it came from Christmas, or perhaps something far sinister...
There are many rumors about it, many of them exaggerated to the point where it's hard to tell fact from fiction. If someone had the physical symptoms in a place like Christmas, then they would probably be avoided/locked up/taken to a hospital.
Halloween? Man, there's crazy things everywhere! The physical signs would most likely be overlooked, as there ARE fog monsters that aren't affected by Insanity at all that live just fine in Halloween. It's the actions/mental health that gives them away, and even then they're more likely to be hospitalized/medicated/not taken seriously as Insanity.

Just a couple of questions about the horsemen insanity. Is it now gone? Or is that up to the players to decide for their individual characters? If it is gone, would Shaheen still bare the markings of it on her skin/hair/eyes?
It is up to the players to decide! However for Horsemen it's just "reset" to stage 1: paranoia, whispers, etc. It's more passive, but Shaheen and other horsemen would still have the marks.
