In my opinion, no ordinary player should EVER hold a True Rune, or it'll be pure insanity. There are only 27 True Runes to around after all, and if anyone can wield a True Rune, we'll be seeing every single Tom, d**k and Harry having the Rune of Punishment or the True Wind Rune. If it ever comes to that, I'll just press that cute little 'quit' button there on the screen.

The reason why we're having so many Suikoden games is so that Konami can space out the appearance of the True Runes. If the MMORPG showcases more True Runes that it known, it might bring the Playstation Suikoden games to a quicker end, which definitely is
not the way we want to go.

I'm assuming here that the Suikoden creators actually have a general storyboard for all the games already, so the history of the entire Suikoden world is pretty much already planned out. Unless there are minor skirmishes throughout the lapses between the games in which the game can be set, it would be highly unlikely that the game will be set in the middle of a war.
Besides, a war between the Nameless Lands/Harmonia (since they're in the North, I believe- what you refer to as Northlands), Zexen Federation/Confederacy (take your pick), Toran, Tinto and the Island Nations would be unlikely, as that would be rather much of a world war- with only the Grasslanders and Falenas opting out. In any case, I suspect that that kind of scene wil only occur in the final Suikoden in which all the other nations wil unite against the Holy Kingdom of Harmonia to overthrow Hikusaak.

Consider me old-fashioned, but I haven't actually played the rest of the Suikoden games save for 1 and 2, in which the Fire Rune doesn't damage you any as your attack never hits your group. Of course, there's an exception in battle if you use a 'Rune' command with Mazus that has the ability to hit everyone within five squares' radius.
Besides, Suikoden isn't that much of a money-earner compared to the more popular Final Fantasy series, so I can't really see them doing an MMOPRG unless the entire series is over and done with- and of course, if there's a demand for it.
