Usually, there are two piercers working together, since it's a tricky piercing. A lot of forceps are used, and a tiiiny needle.
It's actually not the uvula itself that causes you to gag, it's the roof of the mouth. So, an experienced piercer will be able to do the piercing without touching the roof.

Most people accidentally swallow the jewelry within a year though, that's the biggest concern. >:
