I agree with Nat on the picture. Perhaps the 1st picture is not of a grombie, but of a grunny in the process of implanting the virus and the grunny "seed" into a Gaian. Perhaps the picture from the puzzles is the very very first step.
As it is, there would have to be some kind of damage shown for a grunny that has burst out of the skull, don't ya think?
Oh, wait. Look at the picture again. It seems that the skull ends in a crack near the grunny teeth. Am I mistaken in this observation?

 The skull could have easily been bitten through.
If like you said that was a Grunny implanting a "seed" then
you're contridicting yourself.
It can't be implanting a seed AND have grown inside
Then burst out causing the hole.
I did manage to find a level four and convince him to turn around.

I agree with the incubating grunny theory.
Either way, I'm just glad my infection was halted at stage 3.

I had an Infection I was a level 3.
But I got annoyed, and took the vial. Damn it! I should've kept it!

In my opinion, it's likely that the skull was simply bitten through and the grunny in the picture is implanting the seed.
