How would one clamp a surface piercing? put your whole body in a clamp? I've heard of two methods neither of which involve clamps eek

Not usually, no. But I have heard of them being used. I don't think that pic you have would need clamps. There's just five barbells on a fleshy back.

Some piercers do use clamps for a surface piercing, mostly as a guide.
They are usually only used when the skin is clampable, tho.
Josh had his wrist surface piercing clamped ^_^

I had my anti-eyebrow clamped, but when my friend went to get his nape pierced, the piercer did it with an assistant holding up the skin on his neck, essentially. It just really depends on where the piercing is, I'm assuming.

I had a clamp used on both my surface piercings but I know alot of piercers just free hand surface piercings

I like to use them on most surface to make sure of how deep I am piercing. Also, if you do pierce freehand, you need to be sure the person will not move during the piercing.

I have only had punch and taper for surface pierceings. But I dont see how you could use a clamp on a star pattern like that... you would be pulling and hitting the other jewelry.
