Mystearica Fende is her full name.
Didn't they mention it in Abyss?
And Van always calls Tear by that.

The only part I recall them mentioning Gai's full name is when Jade says it after Gai explained his past in Grand Chockmah.

Er... I guess... Maybe she doesn't have one? Because I don't remember anyone mentioning Largo/Badaq's last name... Because well, you know, back then... not all common people actually HAD last names...

Luzu-Kimlasca is her middle name I think.... >> Since she thought she was the princess but was actually Largo's daughter
