well, for starters, I'd make sure it's at least a new string on there. Then I'd find out what method of wrapping your string around the tuning peg. For unwound strings you'd want to use the "reverse biter" method where the string end goes through the hole on the tuning peg, around the opposite way you tune it and up through in front of (more toward the body of the guitar) the rest of the string.
If you're still not satisfied, make sure that the gears inside the tuning machine are turning everything rightly, and that your bridge is free of defects.

the B string is a b***h to get to sound just right. when you tune it up does the needle on the tuner go back and forth? this would indicate that the strings is dischording. If thats the case, try a different strings, make sure its wound right, and make sure it is under any keepers. if all else fails, either find some information on setting your harmonics or take the guitar to get a pro-setup.

Problem: It's a Squier.
Sorry, but its the truth. I'm not saying your guitar is s**t, I'm just saying that Squier has never been known to use the most quality hardware ever. If the above recommendations don't help at all, get yourself a new, decent set of tuning machines.

If the above recommendations don't help at all, get yourself a new, decent set of tuning machines. And/or a new bridge saddle It's probably just me, but if I were going to do something with the bridghe (like replace a saddle), I would just say ******** it and replace the tremelo altogether. A better quality, but still very inexpensive, korean-spaced strat trem can be had from Guitar Fetish (Store.Guitarfetish.com). With this upgrade you get a bigger trem block, which leads to more sustain and better tone, along with the much better quality saddles than what you find on stock Squiers. This coupled with a new set of tuning machines (which can be had from the same place), and you'll definately solve the problem (assuming its not just the brand of strings your using.) And just a personal thing - If it were my guitar, and i liked how it played (good neck, frets, no buzzing, etc), I wouldnt skimp too bad on my new hardware. i.e. I wouldnt just go for the cheapest stuff i could find. If its just a banger guitar until you get the money to upgrade, then just get what you can get by with and save the big bucks for a new guitar down the road.

It may be a problem with the nut, the nut on my guitar is terrible, it pinches the strings which make it a b***h to tune. I think I should probably get a file or something.

If its not that bad you could just apply some graphite dust to the slots to lube'em up. If it is that bad, then widen them the slightest bit (you want them snug with the strings, just not pinching them, and DEFINATELY not letting them wiggle around), then apply the graphite dust.
