It means you would play those strings open and then put your finger on the third fret of those strings immediately after.

"Hammer On", you play whatever string it tells open, then hammer your finger on the string!

to be more precise it is when you see a note says for example 7h8 that means to pick the note on the 7th fret as you normally would then instead of strumming the 8th fret note you instead "hammer on" to the 8th fret with one of your fret fingers instead.
This is theoretically a faster way of playing and how one performs some of those faster solos but it takes time to develop it just right, starting off you might get a drone out of what you hammer on rather than a full clear note. But keep practicing and you should be able to master hammer on's. Btw you could've just looked up this stuff ya know.

Dr. Ghost http://www.youtube.com/watch?v=pRZQGm-_vnY That's quite a good guide, but what he didn;t mention is that you don;t pick each note, just just use your left hand to fret a note.
It doesn;t have to be on an open string, either. Hammer-ons just refer to fretting a note without picking.
It can be used for speed, or just for effect.
