I shall give you a refresher on .hack//SIGN, but next time knowing the anime you need to be refreshed on would help.
Or wait, is this on .hack overall? Overall: There's an MMORPG called The World and the game has a black box within it that causes weird events such as players going comatose to occur.
.hack//SIGN: Tsukasa, a player of The World who wants to escape a nasty reality, has been rendered comatose while his consciousness is trapped inside of The World. Mimiru, Bear, Subaru, and some others wish to help him. Stuff happens. It's a freakin' amazing anime.
