About the one TriEdge371 found, thats when you make it an actual suicide mission, you & your whole squad die. In the end it has Joker talking to the Illusive Man.
To do that horribly I think you would have to make all the wrong choices, and have no upgrades at all.

I never knew you could get this option on Zaeed's loyalty mission. I wonder if you do this if he would survive yet again and come back for revenge in Mass Effect 3.. Doubtful.. But oh would that be awesome.

1. How do you get that assault rifle?
2. My engineer died because I chose Mordin. Do I choose Tali instead?
3. Samara is the best biotic.

1.) Pretty sure it only comes w/ the Collector's Ed. of ME2
2.) Tali and Legion work.
3.) Jack works just as well.
