Follow the format with the title, kay? You can have any format you like within, but it looks rather disorganized (and it gives me a headache) when everyone's got a different format for their journals, and your rp manager is too lazy to go and edit all of them. I've tried; I've edited several quite happily. Now it's your turn.
Use the child's name first. Then use the guardian's name. If you have more than one guardian, as in the case of Milo and Loki, use both their names. So they should look like this
