We don't check every second to see if your okay.
I'm one of those people you can totally tell when they're not happy. I'll either be scowling or generally be giving off this radiation of 'this is so not cool'. I tend to wear my heart on my sleeve, so my emotions are rather clear. Either your girl is a super ninja about her emotions, or you just can't read body language very well.

Ehhh it's a minefield. Some people just outright lie to eachother, and then there's others who are too touchy.
If you misread body language and comment "what's wrong" and nothing is wrong some people blow up. Personally I hate having to guess whether something is wrong because I'm almost always wrong. No one can tell when something is actually wrong with me or whether I'm in a very brief bad mood.
It's very individual and honestly I think the only answer is just... open honesty. Say whatever the hell is bugging is you.

"Yes, you do have to eat the vegetables too" "You just had cookies, you don't need more junkfood today" "Why are your dirty socks under the couch? between the couch cushions? on the floor? under the table? Etc. etc." "Today is garbage day hun, have you put it out?" The list goes on rofl

I'll be honest, I tend to stew on things.
I don't do it because I'm trying to be an a*****e, though, and I'd put down all my money that your girlfriend wasn't either.
I tend to think that I'm overreacting, or that I'm just being neurotic or oversensitive. I don't say anything because I don't feel like I have the right to or that I have good reasoning.
I also don't speak up because I was raised to be respectful and understanding of others. If someone is being mean to me, but is under a lot of stress, I mostly try to brush it off. Sometimes that adds up to frustration and an eventual explosion.
