I noticed in some episodes when Naruto does his sexy jutsu, some of the pervy old men like Ebisu and the Hokage develop a massive nosebleed. Why is this? Is it just a cultural joke, or something? Or do some people really get nosebleeds when they get horny?
Also, since Kakashi reads dirty books, why doesn't he have a constant nosebleed?

Good questions. Good questions.
The whole concept of a Nosebleed happening comes from an old wives tale in Japan where the blood rushes to your face to a point it "bleeds out". You can think of whenever an anime characters gets a nosebleed, the amount of blood is directly proportional to how "horny" he is.
And just look around you. Do you see guys/girls getting nosebleeds just by looking at the opposite/maybe the same sex? If you did, a lot of guys in your school would probably have one at least once a day 3nodding
And to answer your question about Kakashi, he probably just doesn't get aroused by it as much.

As one of the "cool guy" characters, Kishi will never have Kakashi get nosebleeds. Same goes for characters like Itachi, Sai, Sasuke, etc.
That's just the way it is. Like in Chapter 347, Kakashi doesn't really react to Konohamaru's sexy no jutsu thing.

Isn't it obvious that by reading all those dirty books Kakashi has trained himself to no longer lose any blood from his nose when he's excited?

He doesn't get nosebleeds cuz it takes a lot more to get Kakashi worked up blaugh
